Office Policies

We strive to provide a pleasant environment for our guests. We ask that you please refrain from consuming food and drink in our waiting room and clinical areas.

We will always protect our patient’s privacy. Federal law requires that we protect your privacy. We cannot and will not discuss your medical condition, care, etc. with anyone (other than your healthcare providers) without your specific permission.

Payment for medical services is due at the time of service. We are contractually and legally obligated to collect co-payments when treatment is rendered. If planned surgery requires a co-payment, this will also be collected prior to the surgery being performed.

We accept a wide variety of health insurance plans. Please contact your insurance carrier prior to your visit regarding your coverage and co-payment information. If a referral or pre-authorization is required, it is the patient’s responsibility to obtain it prior to the visit. Failure to obtain the referral may require rescheduling the appointment.

Payment plans are available, if necessary, and we also partner with Care Credit as an additional financing option.

Patient safety is one of our highest priorities, so medication refills will only be addressed during normal business hours. We request that our patients always come in to the office if they experience any signs and/or symptoms that they believe may require a new medication.

So that we may meet our patient’s needs we request that if you should need to cancel or re-schedule an appointment that you provide a minimum of 24-hour notice. Please be aware that we reserve the right to charge a fee for an appointment that is not cancelled or rescheduled in advance.
